PSYCHOLOGICAL APPROACH
TO THE TREATMENT OF ONCOLOGICAL DISEASES
One of the actual scientific and practical tasks of medicine is psychological and psychotherapeutic support of oncological patients.
In October 2021, a medical, social and psychological Assistance Service was established in NROC LLP.
The purpose of the service is to provide comprehensive medical, social and psychological assistance to patients and their family members from the moment of applying to NNOC LLP and at all stages of examination, treatment and follow-up in compliance with the principles of phasing and continuity.
Psychological and psychotherapeutic assistance is provided to patients and their family members who have applied to the NROC:
- Psychological counseling and psychological correction.
- Group socio-psychological trainings.
- Psychoprophylactic work in preparation for surgery.
- Psychological rehabilitation of patients who underwent surgery.
- Family counseling.
- Individual and family psychotherapy.
The purpose of psychotherapeutic assistance to patients and their family members is to improve the psychoemotional state of patients through the use of various methods of therapy with creative self-expression with elements of occupational therapy.
Tasks:
- release of suppressed emotions;
- development of creative and communicative abilities of patients.
Most patients with an oncological diagnosis go through
5 main stages of psychological reaction:
1) shock;
2) denial;
3) aggression;
4) depression;
5) acceptance.
1) Shock (fear, anxiety, despair, the possibility of suicide is quite high);
2) Denial (no, not me), repression (I have a banal disease), short and strong, the stage of displacement of the situation. It indicates the unwillingness to take a blow and adapt to stress, "the doctor is wrong, it is necessary to double-check his information", "it is impossible ...".;
3) The stage of aggression (why me?). Time passes and the anxiety that arose at the moment of shock, transfers the patient to the stage of aggression. The same fear underlies it, but it is easier to survive it in an active state, in search of the cause of the disease, the culprit. The patient already realizes that he is sick, searches and finds the causes of the disease, and often blames others for what happened to him.
4) Depression (for what?) — disbelief in the treatment and help of others. It includes not only the experiences of their own despair, longing, hopelessness, but also a sense of guilt in front of relatives whose lives they violated by taking care of themselves. Within the framework of depression, patients often see the disease as a kind of punishment for the wrong life, sometimes they blame fate for injustice and cruelty. Reminding the patient about his condition only increases depression and depression in him. Therefore, it is more appropriate — silence, refusal of verbal consolation;
5) Accepting your destiny. Often, independently or with the help of psychotherapeutic influences, the patient enters the stage of reconciliation with his fate, the phase of acceptance. A person reconciles with his fate, with his illness, feels relieved, reaches mental balance. Most patients die in a state of reconciliation, without fear. However, in the dynamics of reactions to the disease described above, some shifts or repetition of stages are possible. So, after reconciliation with fate, the thirst for life may flare up again, etc. At the stage of "acceptance", a number of authors, as a rule, note the amazing spiritual growth of the patient's personality.
At the stage of primary diagnosis, when the main emotions experienced by patients are anxiety and fear, rational therapy is used to help calm the patient, form a rational attitude to the diagnosis, treatment methods, probable prognoses, and adaptation to the current situation.
With a high degree of anxiety and manifestations of neurosis, hypnotherapy is indicated. Anxiety before surgery, when anxiety
the patient is getting worse, rational psychotherapy and methods of auto-suggestion are also recommended. They are designed to eliminate fear and instill confidence in the success of treatment and a favorable outcome of surgery.
After the operation, the choice of psychotherapeutic treatment methods depends on the patient's well-being and the subsequent plan of therapeutic measures.
We influence our health not only directly, with the help of physical exercises and a certain diet, but also through our ideas, feelings, attitude to life. In addition, the response of the human body to treatment depends on our faith in the effectiveness of treatment and trust in doctors (Karl and Stephanie Simonton, heads of the Dallas Center for Cancer Research and Counseling. Cancer psychotherapy. Recovery from the point of view of a holistic human model). Each person affects both their health and their illness.
Our goal is to show that patients with cancer or other serious diseases can contribute to their recovery, and healthy people can participate in maintaining health.
We specifically use the expressions "influence", "contribute", etc., to emphasize the role that each of us plays in maintaining our own health. Most people believe that healing is something that is done to us, that if we have any health problems, we are only responsible for going to a doctor who will treat us. To some extent, it is. But only to a certain extent.
